Questionnaire answers can be mapped to ICD-10 codes so that selected patient responses automatically create Conditions in the patient's chart when the questionnaire is completed.
ICD-10 mapping is available for:
Yes / No
Multiple choice. Pick one
Select all that apply
Configure Condition Mapping
Go to Administration → Questionnaires and create a new questionnaire or open an existing questionnaire in the Questionnaire Builder.
Add or select one of the supported question types. In the Edit block panel, under Answer options, enable Map to conditions.
Once enabled, a Search ICD-10 code or condition field appears for each answer.
Map Answers to ICD-10 Codes
For each answer that should create a Condition, use Search ICD-10 code or condition to find and select the appropriate ICD-10 code.
Each answer can be linked to one ICD-10 code.
Not every answer needs to have a mapping. An answer can be left without an ICD-10 code when selecting that response should not add a Condition.
Set the Clinical Status
Select the Clinical status that should be assigned to Conditions created from the mapped answers.
The available statuses are:
Active
Recurrence
Relapse
Inactive
Remission
Resolved.
The selected status is applied when the mapped Condition is added to the patient's chart.
Assign the Questionnaire
Once published, the questionnaire can be assigned to a patient.
The assigned questionnaire appears in the patient's Questionnaires section with an in-progress status until it is completed.
Patient Portal
The assigned questionnaire in the patient portal with the In-Progress status.
Patient Completes the Questionnaire
The patient answers the questionnaire normally. The ICD-10 mappings configured in the Questionnaire Builder are not displayed to the patient.
The patient sees the question and its available answer options.
Patient-facing questionnaire
Questionnaire from the patient's perspective with the Yes/No, Multiple Choice, and Select All That Apply examples.
For Select all that apply questions, the patient can select multiple answers. If more than one selected answer has an ICD-10 mapping, each mapped response can be added as a separate Condition.
Review the Completed Questionnaire
After the patient finishes the questionnaire, its status changes to completed.
Staff can open Questionnaire details to review the patient's submitted responses.
The Questionnaire Details view displays the patient's answer labels rather than the underlying ICD-10 mappings.
Completed questionnaire
From practitioners view, in patients Questionnaire, it will show as: completed
Review the Created Conditions
Mapped responses are added to the patient's Conditions using the ICD-10 codes configured in the Questionnaire Builder.
The Conditions panel displays the ICD-10 code, condition name, and configured clinical status.
When multiple mapped answers are selected from a Select all that apply question, each mapped answer is added as a separate Condition.
Conditions created from mapped answers
In our example, the screenshot demonstrates that the mapped Multiple Choice response and both selected answers from the Select All That Apply question were added as separate Active Conditions and showing ICD-10 codes.
Important
Only answers with an ICD-10 mapping generate a mapped Condition. Each answer supports one ICD-10 code, while questions that allow multiple selections can result in multiple conditions when more than one mapped answer is selected.
